Abstract: Controversy about the use of prophylactic probiotic administration in preterm infants for the prevention of necrotizing enterocolitis (NEC) has emerged with strong claims and zealous calls for routine use. Most of the available clinical trials are underpowered for NEC, and meta-analyses of these trials are being used to justify institution of routine prophylactic use of probiotics. Concerns about the current meta-analyses have been raised, and other issues about probiotic safety and efficacy and regulatory issues are discussed in this review.
